What this means for your family

Given the consistent reports of neglect, hygiene issues, and slow response times, we strongly advise families to look elsewhere or conduct an unannounced visit during a weekend or evening shift. If you must use this facility, ensure you have a family member present daily to advocate for basic needs like bathing, clothing management, and medication oversight.

The Timbers Skilled Nursing and Therapy faces significant, recurring criticism regarding patient neglect, poor hygiene, and inadequate staffing levels. While a minority of reviewers report positive rehab experiences, many families describe a facility where call lights go unanswered for hours, residents are left unbathed, and personal belongings frequently go missing. The facility, formerly known as Grace Living Center, is described by several reviewers as having a dark, depressing environment with antiquated communication systems.

Key Review Excerpts

He laid in his urine soaked depends and bed pad for 16 hrs with an open pressure ulcer on his tailbone. Only after speaking with the Director and his Case Worker, were we able to get him the attention he needed.

Daughter of resident · 2022☆☆☆☆

The people we interacted with while my father was there for 3 weeks were all very friendly and nice. However, he was not bathed regularly unless a family member requested it, his sheets were only changed when a family member showed and asked and did so themselves.

Family member · 2023★★☆☆☆

They accepted my mother in law for skilled nursing care knowing she needed to be in isolation due to a contagious infection, and placed her in the memory unit. In the memory hall she constantly has residents walking into her room and using her bathroom.
