About EveryPlace

Our Mission

Finding the right care facility for a loved one is one of the most important — and stressful — decisions a family can make. Many online tools are referral services that only show facilities paying for placement. EveryPlace is different.

We cover tens of thousands of facilities nationwide — nursing homes, assisted living, memory care, and more. Available detail varies by facility type, state, and source, but payment never affects inclusion or result order. Listed facilities do not pay us.

Why EveryPlace Exists

Many senior care directories operate as referral services and receive compensation when a family chooses a participating facility. That model can limit which options a directory presents. EveryPlace is designed as an independent research directory instead.

Meanwhile, the federal government already collects an extraordinary amount of data on Medicare-certified nursing homes: inspection results, staffing levels, quality measures, penalty history, ownership chains, and more. But this data is buried across dozens of spreadsheets and government websites, making it nearly impossible for families to use effectively.

EveryPlace brings all of that data together in one place — organized, searchable, and presented in a way that helps families make informed decisions without hidden agendas.

What Makes Us Different

  Broad coverage, no paid placement. We list tens of thousands of nursing homes, assisted living, memory care, and supported living facilities. No listed facility can pay to rank higher, appear first, or be featured.

  Medicare data made readable. We turn star ratings, inspections, staffing levels, and penalties into plain-language comparisons families can scan quickly.

  Helpful local context. A 3-star rating means more when you can see how nearby options compare. We add that context so each facility is easier to judge.

  Clear review summaries. We summarize public Google reviews to surface repeated strengths, concerns, and questions to ask on a tour.

  Map-first exploration. Location matters. Our interactive map lets you explore covered facilities in your area, filter by type, and quickly zoom into the neighborhoods you care about.

What We Are Not

EveryPlace is not a referral service. We do not accept placement fees, advertising, or pay-for-ranking from any facility. We do not sell your personal information. Payment never influences result order; the source metrics we display come from government records and public Google business information and reviews.

We are also not a healthcare provider or advisor. The information on EveryPlace is for research and comparison purposes. We strongly encourage families to visit facilities in person, talk to current residents and their families, and consult with healthcare professionals before making a decision.

Our Data

We combine multiple public and publicly accessible data sources to build a broader research picture of each facility where those sources are available:

Medicare nursing home data

For nursing homes, we use data from the Medicare Care Compare:

• Overall quality star ratings (1–5 stars)
• Health inspection results
• Staffing levels (RN hours, total nurse hours)
• Quality measure scores
• Penalty and fine history
• Ownership and chain affiliation data

State Licensing Data

For assisted living and memory care, we pull available inspection records from state licensing agencies including California CCLD, Washington DSHS, Iowa DIAL, Colorado CDPHE, and more. The level of detail and availability varies by agency.

Google reviews and summaries

Where public Google review data is available, we use it to show ratings, recurring themes, review excerpts, owner-response patterns, and suggested tour questions. AI-generated summaries are labeled and should be checked against the original reviews.

How Ratings Work

CMS star ratings on EveryPlace come directly from Medicare and are labeled as such. The overall CMS rating combines health inspections, staffing, and quality measures. Public Google ratings are labeled separately. We do not create a proprietary star rating; we present source ratings alongside the underlying data so families can compare them in context.

We also add local comparison context so you can see how a facility compares with others in the same state.

EveryPlace is built by Prismind Analytics, LLC, a data analytics company focused on making public data accessible and actionable for everyday decisions. We believe that transparent, independent information supports better research by families and accountability for care providers.
