EveryPlace is a free, publicly accessible platform that aggregates and presents data about Medicare-certified nursing homes and senior living facilities in the United States. Our data is sourced primarily from the Centers for Medicare & Medicaid Services (CMS) and supplemented with publicly available information such as Google reviews.

The Site is provided for informational purposes only. It is not a healthcare provider, medical advisor, or placement/referral service.

3. Not Medical or Professional Advice

The information on EveryPlace is for general informational purposes and does not constitute medical, legal, financial, or professional advice. The data presented, including star ratings, inspection results, staffing levels, and quality measures, is derived from publicly available government sources and may not reflect the most current conditions at any facility.

You should not rely solely on information from the Site when making decisions about senior care. We strongly recommend visiting facilities in person, consulting with healthcare professionals, and contacting your local ombudsman program before making a care decision.

4. No Referral or Placement Services

EveryPlace does not provide referral or placement services. We do not receive fees, commissions, or compensation of any kind from facilities listed on the Site. Facility listings are not endorsements or recommendations. The presence or absence of a facility on the Site does not indicate any evaluation, approval, or endorsement by us.

5. Data Accuracy and Limitations

While we strive to present accurate and up-to-date information, we make no warranties or representations regarding the completeness, accuracy, reliability, or timeliness of the data on the Site. Specific limitations include:

• CMS data is updated periodically and may lag behind actual facility conditions by weeks or months.
• Star ratings reflect a point-in-time assessment and may not capture recent improvements or declines.
• Staffing data is self-reported by facilities and subject to verification by CMS.
• Google reviews reflect individual opinions and experiences and are not verified for accuracy.
• Facility contact information, addresses, and operational details may change without notice.

6. Intellectual Property

The Site's design, layout, code, graphics, logos, and original content are owned by Prismind Analytics, LLC and protected by copyright and other intellectual property laws. The underlying facility data sourced from CMS is public domain and is not subject to our copyright claims. Our compilation, analysis, presentation, and benchmarking of that data is proprietary.

7. Acceptable Use

You agree not to:

• Use the Site for any unlawful purpose or in violation of any applicable laws or regulations
• Scrape, crawl, or use automated tools to extract data from the Site in bulk without our written permission
• Attempt to interfere with, compromise, or disrupt the Site or its infrastructure
• Impersonate any person or entity or misrepresent your affiliation
• Use the Site to harass, defame, or harm any individual or facility
• Reproduce or redistribute the Site's proprietary analysis or benchmarks for commercial purposes without permission

TO THE FULLEST EXTENT PERMITTED BY LAW, PRISMIND ANALYTICS, LLC AND ITS OFFICERS, DIRECTORS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, INCLUDING BUT NOT LIMITED TO LOSS OF PROFITS, DATA, USE, OR GOODWILL, ARISING OUT OF OR RELATED TO YOUR USE OF OR INABILITY TO USE THE SITE.

IN NO EVENT SHALL OUR TOTAL LIABILITY EXCEED ONE HUNDRED DOLLARS ($100.00). THIS LIMITATION APPLIES WHETHER THE CLAIM IS BASED ON WARRANTY, CONTRACT, TORT, OR ANY OTHER LEGAL THEORY.

11. Indemnification

You agree to indemnify, defend, and hold harmless Prismind Analytics, LLC and its officers, directors, employees, and agents from and against any claims, damages, losses, liabilities, and expenses (including reasonable attorneys' fees) arising out of or related to your use of the Site or violation of these Terms.
