1. About this directory
EveryPlace.care is an independent research directory that aggregates publicly available information about licensed long-term care facilities in the United States. It is operated by Prismind Analytics, LLC.
We are not affiliated with, endorsed by, sponsored by, or operating under any commercial arrangement with any listed facility. We do not accept payment for placement, do not collect referral fees, and do not sell leads. A listing is not an advertisement, endorsement, or recommendation — it is a reference entry.
2. Where our data comes from
Facility information on EveryPlace is compiled from public-record and publicly accessible sources, including:
- CMS Care Compare and Medicare.gov — federal quality ratings, staffing data, deficiency citations, and penalty histories for nursing homes.
- State licensing portals — including the Texas HHSC Unified Licensure Information Portal (TULIP), the California Community Care Licensing Division, New York DOH Profiles, and analogous portals operated by other state regulators.
- Google Places — facility name, address, contact details, photos, and user-submitted reviews.
Each facility page links back to the underlying public records so that visitors can verify the source for themselves.
3. About the photos on our site
All facility imagery on EveryPlace comes from public sources — primarily Google Places photos and Google Street View. We do not take, commission, or upload our own facility photographs. The images displayed on any facility page reflect what is already publicly viewable through Google’s services for that address.
4. Factual corrections
We welcome factual corrections from facility operators, residents, families, and the public. Corrections apply to factual errors in the listing itself — for example, an incorrect phone number, an outdated address, a misattributed photo, or an incorrect facility type.
Please send factual corrections through our contact email and identify the specific item to be corrected and, where relevant, the source that supports the correction. Typical turnaround is a few business days.
5. Disputes about underlying public records
If a dispute concerns what state inspectors or federal regulators found — rather than how we display it — the correct path is the issuing agency’s correction or appeal process. State licensing agencies and CMS each maintain procedures for providers to challenge or supplement the official record.
EveryPlace does not edit, redact, or suppress regulatory findings in response to operator-side disputes. If the underlying public record is amended by the issuing agency, our listing will reflect that change when we next refresh from the source.
6. Photos and street-level imagery — courtesy removal
Many long-term care facilities are licensed as commercial businesses operating from non-residential locations, and we display public-record imagery of those locations accordingly. For small residential-licensed operators — for example, a licensed care home operating from a residential structure — we will consider requests to remove street-level imagery from the listing as a courtesy.
The facility’s name and licensed business address, which are sourced from public state licensing records, will remain on the listing.
7. Trademark and nominative use
A directory entry that identifies a business by its lawful name for the purpose of referring to that business is nominative fair use, not trademark infringement. EveryPlace does not claim affiliation with, sponsorship by, or endorsement from any listed facility, and no such relationship is represented or implied by the inclusion of a facility’s name in the directory.
8. Legal correspondence
Operational matters — factual corrections, image-removal requests, and similar — should be sent through our contact email.
Legal correspondence should be directed by counsel to legal@everyplace.care.
9. Updates to this policy
We may update this Research Policy from time to time. Changes will be posted on this page with an updated “Last updated” date.
