What this means for your family

While the facility offers strong rehab therapy and engaging community events, the recurring reports of poor sanitation and inconsistent nursing care are major red flags. If you are considering this facility, perform an unannounced visit to check the cleanliness of patient rooms and observe staff-to-patient interactions during off-peak hours.

Questions for Your Tour

  • 1Given that the facility has 150 residents, what specific protocols are in place to ensure that nursing and aide staff remain responsive to individual needs throughout the day and night?
  • 2I noticed some concerns regarding room maintenance; could you walk me through your daily housekeeping schedule and how you ensure rooms remain sanitary and odor-free?
  • 3I see that physical and occupational therapy are strong points here; how do you integrate these therapy sessions into the residents' daily schedules to ensure they don't conflict with meal times?
  • 4Could you describe the process for monitoring and improving the quality and consistency of meal service to ensure residents receive their nutrition on time?
  • 5With recent state inspections highlighting areas for improvement, what specific steps is the leadership team taking to address these findings and enhance the overall standard of care?
  • 6I’ve heard great things about your organized events; how do you encourage residents to participate in these activities, and are there options for those who prefer more low-key social engagement?

Key Review Excerpts

The physical and occupational therapy is outstanding. I would not refer this place for long term care they are always understaffed and the patient rooms have roaches!!!!

Visitor · 2024★★★★★

On first look entering the building it meets the eye test in appearance/cleanliness. Employees were pleasant and helpful. But, it was not the same environment upon entering Mr. Black’s room. It had a strong smell of urine and his sheets were dirty.
