Avenue Care and Rehabilitation Center receives highly polarized feedback, with many families reporting severe neglect, understaffing, and poor hygiene, while others praise specific staff members, particularly in the activities department. While the facility is physically attractive, multiple reviewers describe a systemic failure in basic patient care, including delayed medication, unanswered call lights, and unsanitary room conditions. Key Review Excerpts

My sister had reported that she had soiled linens to her aide and I had to call the DON and report the issue twice before they rectified the issue. She also complained about not getting her pain medication for almost two hours.

Long-term resident's family · 2025☆☆☆☆

The facility looks good, but the care is far from it patients in bed for long hours with bed sores, the facility don't have proper sized depends for some bigger patients so instead they use cheap quality diapers that fail after applying them on residents, call lights go unanswered for more than 10-15 minutes.

Family member · 2026☆☆☆☆

They receive 3 stars… the 1st star goes to the activities center. They go above and beyond to serve the residents and even take them on enjoyable outings whether they are wheelchair bound or not. The 2nd star goes to the therapy division.
