What this means for your family

While the facility is physically modern and offers strong therapy services, the high frequency of reports regarding ignored call buttons and neglect is a major red flag. If you consider this facility, we strongly recommend unannounced visits during weekends and evenings to verify staffing levels and response times yourself.

Key Review Excerpts

The therapy department is fantastic, nursing staff is very knowledgeable and responsive, and the administrative is top notch.

Rehab patient family · 2016★★★★★

My grandmother was there for 2 weeks. Nice facility food was decent but staff was terrible. My grandmother fell in her bathroom while i was there. I went to nurses station amd told the nurse sitting there she had fallen and needed help. She said im not the nurse on duty and pointed to someone down the hall.

Grandchild of resident · 2016☆☆☆☆

The staff is extremely inattentive and uncaring. My Mom was there for several weeks and my sister and I were appalled. One of their 'dinners' consisted of a fried bologna sandwich and cheese puffs.

Daughter of resident · 2023☆☆☆☆

Families have repeatedly filed complaints against this facility, triggering 27 of its 54 total deficiencies across multiple surveys. The most recurring problems involve medication management errors, nutrition and dietary issues, and resident rights violations. While the facility corrects cited deficiencies, the pattern of repeated complaints in the same care areas—including medication errors, inadequate food service, and resident care failures—suggests ongoing quality concerns that families should carefully evaluate before placement.
