Resident A experienced stroke symptoms (facial drooping, arm weakness, leg weakness) on 3/4/2024, and the facility did not seek medical attention because the POA refused; however, the facility failed to ensure the resident received adequate and appropriate care/information regarding medical condition.

Resident A was lifted inappropriately by staff using the Hoyer Lift, resulting in Resident A's head banging on a wall and a small scratch on his right leg.
