What this means for your family

While North Bank has seen recent improvements in facility aesthetics and physical therapy outcomes, the recurring reports of neglect and medication errors are serious red flags. If you choose this facility, we strongly recommend daily, unannounced visits to monitor your loved one's hygiene and medication administration, and maintaining direct, frequent communication with the administrative team.

Key Review Excerpts

My aunt came to Northbank unable to walk and do for herself, my aunt left WALKING out of the facility and was able to go home, I can’t thank the staff enough at Northbank for being consistent and helping her reach her goals.

Niece of rehab patient · 2025★★★★★

My father has passed away from the abuse and neglect of this horrible place h3 lost 100 pounds in 8 months and they left him in bed for months at a time he just passed away from a surgery from an deep infection on a very deep bed sore.

Daughter of deceased resident · 2025☆☆☆☆

The floor manager would not give him his medication and denied him wound bandages. This lady needs to be reprimanded. She even lied and told me that they would take him to his appointments. They never took him to one single appointment he had.

5deficiencies
Near state avg (7.3)
1 complaint-triggered

This facility has a concerning pattern of recurring safety and care issues, with families filing at least one complaint that triggered an investigation regarding abuse reporting procedures. The most frequent problems involve fire safety systems, medication management, and overall quality of care including wound care and accident prevention. While all deficiencies show correction dates, the persistence of similar issues across multiple surveys from 2021 to 2025 suggests ongoing operational challenges that families should carefully consider.
