What this means for your family

Morgan Health Center has a strong reputation for its clean, modern environment and a highly engaged administrative team that many families find supportive. However, you must be vigilant regarding basic care; multiple families reported issues with call-bell response times and hygiene. If you choose this facility, establish a clear communication plan with the nursing supervisor and visit at varied, unannounced times to ensure your loved one's needs are being met consistently.

Morgan Health Center receives highly polarized feedback, with many families praising the facility's cleanliness, recent renovations, and the dedicated efforts of specific staff members like Mario. However, a significant number of reviewers report serious concerns regarding neglect, including slow response times to call bells, poor hygiene, and inadequate communication regarding medical changes. While many families feel their loved ones are safe and well-cared for, others describe a facility that is understaffed and unresponsive to basic patient needs.

  • 1I noticed in your recent responses to families that you value transparency; how do you currently keep families updated on changes in a resident's medical status or care plan?
  • 2Given that staffing levels are a priority for everyone, what specific protocols do you have in place to ensure call bells are answered promptly during peak hours?
  • 3We read that your therapy programs are quite strong, but could you walk us through how the team ensures consistent daily hygiene and personal care assistance for residents?
  • 4With the current CMS staffing rating, how does the facility manage staff-to-resident ratios to ensure that every resident receives the supervision and attention they need throughout the day?
  • 5What steps are being taken to improve the dining experience and nutritional variety, as we understand this is an area the facility is currently working to enhance?
  • 6How does your team handle medical emergencies or urgent health changes during the evening and weekend shifts to ensure residents are safe and families are informed?

Key Review Excerpts

My dad was in the rehab for a few weeks and the staff and Mario were exceptional with his care. My dad was happier, more communicative and more talkative than he was doing in his assisted living facility prior to entering Morgan.

Memory care family member · 2025★★★★★

The staff are nice but that’s where it stops. This facility has an active Covid outbreak 12/6/24 on the second floor and never notified my mothers case management team at Rhode Island hospital nor did they notify me her POA.

Long-term resident's family · 2024☆☆☆☆

My wife waited 11 damn hours for a drink of water. My wife was supposed to get 5 days of PT & OT and received only 1 day of each. Some of the staff need to be trained in how to interact with the public as some of them are rude.

This facility has a concerning pattern with 21 of 32 deficiencies triggered by family complaints, indicating ongoing problems that required external intervention. The most recurring issues involve medication management, basic care quality, and infection control, with medication errors and pharmacy service problems appearing repeatedly across multiple surveys. While all deficiencies show correction dates, the persistent nature of similar problems—particularly medication safety issues appearing from 2023 through 2025—suggests systemic challenges that families should carefully consider before choosing this facility.
