What this means for your family

While Mentor Ridge offers a beautiful, modern environment and highly effective physical therapy, the frequency of reports regarding neglect and slow response times is concerning. If you choose this facility, we strongly recommend frequent, unannounced visits and maintaining a very active role in your loved one's daily care and communication with the nursing staff.

Key Review Excerpts

My dad was recently here after he had several falls at home. The staff was so kind to him and made him feel at ease. The place was clean and the food was pretty good for a facility.

Long-term resident's family · 2025★★★★★

My mom went to Mentor Ridge for in-patient rehab after a knee replacement, and we couldn't have been happier with the care she received. Not only is the facility beautiful, well maintained and immaculate, but the occupational and physical therapy was exactly what she needed.

Rehab patient's family · 2018☆☆☆☆

Morning we pulled my mom out of this facility within 48 hours. Everything they said that they could do they did not. Our biggest issues: Only one nurse all night and no one came into check on her the ENTIRE night.
