Key Review Excerpts

The rooms were so nasty! Exposed wires, blood stains, food stains, the place smells HORRIBLE, they do not take care of the residents letting them sit in their own

Memory care family member · 2023☆☆☆☆

NO ONE SHOULD HAVE TO WAIT OVER 4 HOURS TO HAVE THEIR CALL LIGHT ANSWERED! Oh yes, someone comes in to turn off the light and say your Aide is at lunch and will be here when she returns.

Family member · 2021★★★★

The physical therapists and occupational therapists were wonderful. Some of the nurses and some of the CNA's (especially Bernice) did a good job.

40 complaint-triggered

Families have filed numerous complaints triggering many of the 95 deficiencies found across surveys, with recurring issues in daily care assistance, medication management, and facility cleanliness/safety. The facility shows persistent problems with pressure ulcer care, infection control, and medication errors that appear across multiple surveys from 2022 to 2025. While all deficiencies show correction dates, the repeated nature of similar violations suggests ongoing care quality challenges families should carefully consider.
