What this means for your family

The facility's rehabilitation department is frequently praised for its effectiveness, making it a potential option for short-term recovery. However, given the recent, serious concerns regarding nursing responsiveness, sanitation, and administrative communication, families should prioritize an in-person visit and ask specifically about current staffing ratios and how they handle patient hygiene and dietary requests.

Key Review Excerpts

Since she has been there, they have gotten her sugars under control to the point where she doesn't need meds anymore and have been successful in getting her to want to get out of bed to get her hair done and other things for herself.

Memory care family member · 2025☆☆☆☆

The physical therapy services and occupational therapy services have been outstanding. The nursing care has been equally outstanding. All staff from admissions, social services, activities, kitchen services etc. are caring, supportive, efficient and empathetic.

Long-term resident's family · 2023★★★★★

My family had a disappointing experience with this facility. My family member was only there a month, the lack of care and attention to basic patient needs were enough to upset us. In addition there was no communication from any social worker or medical progress reports given to the family.

Families have filed complaints triggering investigations, including serious concerns about resident protection from abuse and neglect, and pressure ulcer care issues. The facility shows persistent problems across multiple care areas including infection control, medication management, food service, and resident safety protocols. While all deficiencies appear to have correction dates, the pattern of recurring issues in similar areas across surveys suggests ongoing challenges with consistent care quality and safety standards.
