What this means for your family

While the facility has a strong reputation for its respiratory and physical therapy programs, the recurring reports of neglect and slow response times are significant red flags. If you choose this facility, we strongly recommend conducting unannounced visits and specifically asking the administration about their weekend staffing ratios and protocols for call-light response.

Key Review Excerpts

The CNA are amazing but unfortunately they are overworked so not matter what they do it can't justify the lack of integrity that is displayed.

Rehab patient · 2025☆☆☆☆

My 68 y/o mother was a patient here because she needs a tracheotomy and ventilator care. Most recently she was admitted to St. Francis hospital on 1/26/23 due to: RIGHT HUMERUS FRACTURE and RIGHT FEMORAL NECK FRACTURE.

Memory care family member · 2023☆☆☆☆

The staff here is pretty good and there is one who deserves great praise his name is Michael J. He's a manager but also helps CNAs. I've had him dozens of times and he took great care of me.

$234,598 in fines

This facility shows a concerning pattern of repeated complaint-triggered deficiencies, with families filing reports that led to over half of the 98 total violations found across 22 surveys. The most recurring problems involve accident prevention and safety supervision, abuse and neglect protection, and medication management. While all deficiencies show correction dates, the persistent nature of safety issues and multiple abuse/neglect citations spanning from 2023 through 2025 suggests ongoing care quality challenges that warrant careful consideration before placement.
