What this means for your family

While many families report a clean environment and caring leadership, there is a recurring pattern of serious safety concerns regarding falls and medical neglect. We strongly advise you to conduct unannounced visits during weekends or evenings to verify staffing levels and to ask specifically about their fall prevention protocols and how they communicate medical changes to families.

Eagle Valley Meadows receives highly polarized feedback, with many reviewers praising the facility's cleanliness, fresh environment, and the dedication of specific staff members like the Executive Director. However, a significant number of families report severe concerns regarding patient neglect, including unaddressed falls, dehydration, malnutrition, and poor communication during medical emergencies. Prospective families should weigh the positive reports of a welcoming atmosphere against the recurring, serious allegations of inadequate care and staffing shortages.

Key Review Excerpts

She was admitted at the weight of 70-75lbs and had gotten down to 57 lbs. She fell at this facility repeatedly! They stated, 'she has the right to fall. There is nothing they can do about it.'

Memory care family member · 2024☆☆☆☆

I was adament that my mother would NEVER be placed in a facility. After an extended hospital stay, it was obvious that she would need rehab prior to makingnit home. We, unfortunatley, had 3 admissions over the summer as mom struggled with her health. Thankfully, her care team recognized her need for hospital care and was able to get her transferred quickly.

Long-term resident's family · 2024★★★★★

On his 8th day, he was rushed to the emergency room near death. He's still in the hospital where they are trying to undo the damaged he suffered from neglect

Eagle Valley Meadows has concerning patterns with families filing 18 complaint-triggered deficiencies across multiple surveys. Recurring issues include medication management, infection control, and resident safety hazards appearing repeatedly from 2023 through 2025. While the facility corrects violations when cited, the persistence of similar problems—particularly around daily care assistance, nursing competency, and maintaining safe environments—suggests ongoing operational challenges that warrant careful evaluation during any visit.
