Key Review Excerpts

The physical and occupational therapy staff are great. They are very friendly and work well with my dad. They also spent time showing us what we can do to help him outside of the physical/occupational therapy sessions.

Long-term resident's family · 2026★★★★★

For the most part staff and admin are good but have become blind and complacent when it comes to how things are done in the dementia unit. They trap the residents in the dining room because they don’t have the staff to adequately watch over their safety.

Memory care family member · 2024☆☆☆☆

The Rehab here is amazing! I prayed and prayed for the right facility for my son, and I knew we would be placed at the right one. Many others denied him but he was accepted here and I had few expectations for his progress.

Families have filed multiple complaints leading to recent safety violations, including two accident hazard citations in 2025. The facility shows recurring problems with safety and accident prevention, fire safety systems, and resident care assistance that persist across multiple surveys from 2022-2025. While all deficiencies show correction dates, the pattern of repeated violations in critical areas like resident safety suggests ongoing operational challenges that warrant careful consideration.
