What this means for your family

While the rehab therapy team is highly regarded, the facility has a concerning pattern of neglect and poor nursing response times. Families should be extremely cautious; if you choose this facility, ensure you have a daily presence to monitor hygiene, call light response, and medication administration, as management has been reported to be unresponsive to concerns.

Brookdale University Park SNF presents a stark contrast between its modern, aesthetically pleasing facility and significant, recurring failures in patient care. While many reviewers praise the rehab therapy team and dietary staff, a large number of families report serious issues including neglect, slow response times to call lights, poor hygiene, and unprofessional conduct by nursing staff. Potential families should be aware that while the building is beautiful, multiple reports of safety concerns and communication breakdowns suggest a high-risk environment for vulnerable residents.

Questions for Your Tour

  • 1Given the recent focus on staffing, what specific protocols are in place to ensure call lights are answered promptly and resident requests for assistance are addressed in a timely manner?
  • 2I noticed the facility has a strong reputation for therapy and dietary services; how do you ensure that same level of attentive, compassionate care is consistently applied to daily hygiene and personal grooming needs?
  • 3With the current CMS health inspection ratings, what concrete steps is management taking to improve oversight and ensure all safety and medication administration protocols are strictly followed?
  • 4How does your leadership team facilitate communication with families, and what is your process for providing timely updates regarding a resident's health status or any changes in their care plan?
  • 5I see that you engage with feedback online; how do you translate that family input into actual changes or improvements within the nursing staff's daily routines?
  • 6What does a typical day look like for residents here, and how do you encourage social engagement while balancing the need for close medical monitoring?

Key Review Excerpts

She was left in a hallway alone and fell out of her wheel chair which damaged her arm to the point it was no longer useful. She was assaulted by a male custodian in her room( I had a bear camera installed after that) and was never properly attended to.

Long-term resident's family · 2025☆☆☆☆

My mother was here for skilled nursing and rehab for 20 days. With the exception of a couple of nurses the care was awful. She was yelled at by staff to get up and go to bathroom with no assistance and some help was ugly to her.

Long-term resident's family · 2025☆☆☆☆

The rehab personnel is wonderful. But some of the people on the floor are terrible want keep her changed serving food while she's wet want bath her on the days she's suppose to be bathed
