Key Review Excerpts

The therapists bring you back to life. The really push you to get better. The Activity department is the best and gets a 10.

Rehab patient · 2025★★★★★

Every time I went to visit her, she was soiled, not checked on even after she would press the nurse call button and would tell me they were very rude to her when visitors weren't around.

Long-term resident's family · 2025☆☆☆☆

My dad was their for 1 day and I got him up out of there. The staff that I met was rude, unprofessional and to top that off they gave my dad a broken wheelchair and he is an amputee.

This facility shows serious concerns with persistent safety and care issues that have prompted numerous family complaints. The most recurring problems involve accident prevention and supervision, basic daily care assistance, and medication management. While all deficiencies show correction dates, the pattern of repeated violations across the same care areas—particularly safety hazards appearing in complaint investigations from 2023 through 2025—suggests ongoing systemic problems that families should carefully evaluate before visiting.
