Key Review Excerpts

The staff takes an excessive amount of time to respond to call lights—even for basic needs. My father’s room is often dirty, and essential supplies like gowns, bed pads, and footies are frequently unavailable.

Memory care family member · 2025☆☆☆☆

My family’s items are CONSTANTLY coming up missing & it’s NOT THE RESIDENTS (im told) and I BELIEVE IT. The employees are helping themselves to the defenseless residents food, snacks, shoes, clothes, phones, phone chargers.

$323,373 in fines

This facility has concerning patterns of family complaints, with 58 out of 99 deficiencies triggered by reports from families or advocates. The most recurring problems involve protection from neglect and abuse, accident prevention and safety supervision, and wound care for pressure sores. While all violations show correction dates, the persistence of similar issues across multiple surveys - particularly safety hazards appearing repeatedly from 2023 through 2025 - suggests ongoing challenges in maintaining consistent quality care standards.
