The facility failed to ensure the accuracy of the electronic medication administration record (eMAR) for three sampled residents. Specifically, for Resident #1, there were multiple instances where doses of APAP, atorvastatin, and carvedilol were entered without documentation of whether they were administered or the reason for omission.
