Additionally, residents were unable to consume the provided baby carrots due to difficulty chewing raw vegetables.

The facility failed to ensure residents received a complete set of flatware, including a knife, fork, and spoon, during meals. Observations during lunch showed place settings only included a fork and a napkin, despite knives being available in the kitchen. Interviews with staff and residents confirmed that full place settings were not standardly provided unless specifically requested.
