Specifically, during lunch service, the facility substituted potato chips for celery, a banana, and vanilla pudding without documenting the change in the substitution log. Additionally, residents were unable to consume the provided baby carrots due to difficulty chewing raw vegetables.

The facility failed to ensure residents received a complete set of flatware, including a knife, fork, and spoon, during meals. Observations during lunch showed place settings only included a fork and a napkin, despite knives being available in the kitchen. Interviews with staff and residents confirmed that full place settings were not standardly provided unless specifically requested.
