Key Review Excerpts

I don't know if Deanna Dang works at this facility anymore, and it's been 11 years since my father died, but this was the ONLY place I went to that had staff that outwardly showed they cared about the people they were taking care of.

Long-term resident's family · 2021★★★★★

Staff and administrators have been helpful and caring. The facility is in a pretty and wooded setting. Read some reviews that the building had a damp smell. It looks like several venting features were added and this does not seem to be a problem any longer.

9deficiencies
Near state avg (12.0)
1 complaint-triggered

AHVA Care of Winfield has had 28 federal deficiencies across four surveys, including one complaint-triggered investigation for resident abuse and neglect. The facility shows recurring problems in three main areas: quality of care (including wound care, mobility assistance, and medical treatment), medication management, and fire safety systems. While all deficiencies show correction dates, the pattern of repeated issues across multiple surveys suggests ongoing operational challenges that families should carefully consider.
