Specifically, for 3 of 3 sampled residents, the person administering the medication did not provide the required initials or signature.

The facility failed to maintain hot water temperatures within the required range of 100 to 116 degrees Fahrenheit in three resident bathrooms. Observations showed temperatures as high as 124 degrees Fahrenheit. Additionally, the Supervisor-In-Charge was unaware of the required temperature range and could not provide consistent temperature monitoring logs.

The facility failed to maintain hot water temperatures between 110 degrees F and 116 degrees F in three resident-accessible bathrooms. Specifically, temperatures in bathrooms #1 and #2 were recorded as high as 124 degrees F in the shower and 122 degrees F in the sink.
