The facility failed to ensure that Medication Administration Records (MAR) were accurate for three sampled residents. Specifically, the records did not properly document the name or initials of the person administering the medications.

The facility failed to ensure that Medication Administration Records (MAR) were accurate regarding the documentation of the name or initials of the person administering medications. Specifically, for 3 of 3 sampled residents, the person administering the medication did not provide the required initials or signature.

The facility failed to maintain hot water temperatures between 110 degrees F and 116 degrees F in three resident bathrooms. Specifically, temperatures were recorded as high as 124 degrees F in the shower and sink of bathroom #1, and 122 degrees F in both the shower and sink of bathroom #2.

The facility failed to maintain hot water temperatures within the required range of 100 to 116 degrees Fahrenheit in three resident bathrooms. Observations showed temperatures as high as 124 degrees Fahrenheit. Additionally, the Supervisor-In-Charge was unaware of the required temperature range and could not provide consistent temperature monitoring logs.
