Key Review Excerpts

The rehab department is a standout, led by a compassionate director and staff who genuinely care for patients' success. The CNAs are also fantastic, working efficiently and compassionately despite heavy workloads.

Long-term resident's family · 2025★★★☆☆

I’ve always used to say this place was an F I would’ve graded at that button since Shari Has taken over this place is a A+. I am grateful for her because she has made a different change in this facility.

Memory care family member · 2025★★☆☆☆

My mom was there for about 90 minutes. Dried “stuff” under the next bed, giant roaches, dirty window sills. I said something about the roaches and the nurse said “yea, I ain’t gonna lie, we have a big problem”.

1 complaint-triggered

One family filed a complaint that led to citation for medication labeling issues. The facility shows recurring problems with daily care assistance, emergency preparedness, and medication management across multiple surveys from 2021 to 2025. While all deficiencies have documented correction dates, the persistence of similar issues over four years, including repeated citations for basic resident care and respiratory services, suggests ongoing operational challenges families should investigate during their visit.
