Key Review Excerpts

The staff is the heart of this facility—skilled, compassionate, and always attentive to residents’ needs. From day one, we felt welcomed and supported.

Family member · 2025★★★★★

My dad was supposed to be in rehabilitation after a hip surgery and when he came home he had BAD untreated sores on his 'behind' and a huge one on the heel as if he was moved from bed very little.

Family member · 2022☆☆☆☆

The Rehab was okay, but the shower room is caked with dried feces and Roaches.

This facility shows concerning patterns with multiple family complaints triggering 15 of its 35 deficiencies, including serious issues with abuse protection that recurred in 2023 and 2024. The most frequent problems involve environmental safety, resident rights violations, and care quality issues. While the facility has correction dates for all deficiencies, the repeated nature of abuse protection failures and ongoing complaint-driven investigations suggest families should carefully evaluate whether this facility can consistently provide safe, respectful care.
