The inspection was a Case Management visit conducted on February 26, 2026. The Licensing Program Analyst reviewed records and followed up on the facility's fire clearance and make-up air unit system. No deficiencies were issued during the visit.

The primary finding relates to the failure to generate and submit an Unusual Incident Report within the required 7-day reporting period following an incident that occurred on 11/11/25. One Type B deficiency was cited regarding the availability of the incident report for review.

The complaint investigation found the allegation regarding safeguarding resident belongings to be substantiated. The primary deficiency cited relates to inadequate record-keeping for resident valuables. The facility must correct this by ensuring all residents have a properly completed and signed Safeguard For Property Values form on file.

The visit was a Case Management review prompted by a complaint investigation regarding staff intoxication. The Licensing Program Analyst interviewed the administrator, staff, and residents. No evidence was found to substantiate the allegations, and consequently, no deficiencies were issued.
