This was an unannounced Case Management visit conducted for a Death Report (DR) related to Resident 1 (R1). The Licensing Program Analyst met with the Business Office Director to clarify documentation needed for the report. No deficiencies were cited during today's visit.
The facility underwent a required annual inspection on 05/22/2025. The LPA observed the facility to be generally compliant, noting proper supplies, functioning equipment, and current resident documentation. No deficiencies were cited during this visit.
