COMPLAINT INVESTIGATION REPORT

The investigation found insufficient evidence to prove or disprove the allegation, resulting in the finding that the allegations are UNSUBSTANTIATED. No deficiencies were issued during this complaint visit.

The inspection revealed two critical deficiencies. First, the facility failed to maintain a comfortable temperature for residents, and second, full bed rails were observed on a resident who was not on hospice care. Immediate corrective action is required for both temperature control and bed rail compliance.

The Annual inspection was conducted on 05/17/2024. The facility was generally observed to be well-maintained regarding food storage, medication security, and general cleanliness. However, one Type B deficiency was cited related to maintenance issues, specifically broken furniture and a malfunctioning closet door, which poses a potential safety risk.
