What this means for your family

This facility shows strong results for short-term rehabilitation and physical therapy, which may be beneficial for post-operative recovery. However, given the reports of slow call-light response times and concerns regarding discharge planning, we strongly recommend visiting during different times of the day to observe staffing levels firsthand and asking specifically about their process for coordinating post-discharge medical supplies.

Google Reviews

Google Reviews

Timber Springs Transitional Care receives polarized feedback, with some families praising the compassionate nursing staff and effective therapy programs, while others report significant concerns regarding staffing levels and clinical oversight. While several reviewers highlight a clean environment and dedicated care, there are serious allegations regarding inadequate discharge planning and slow response times for call lights. Families should weigh the reported success in rehabilitation against the inconsistent communication and potential understaffing noted by daily visitors.

Questions for Your Tour

  • 1Given the current staffing levels, what is your specific protocol for ensuring call lights are answered promptly, especially during shift changes?
  • 2How do you ensure consistent communication between the nursing team and family members regarding changes in a resident's medical status or medication adjustments?
  • 3I understand you have a strong reputation for rehabilitation; how do you integrate those therapy goals into the daily routine for residents who are here for longer-term care?
  • 4What steps is the leadership team currently taking to address the recent CMS inspection findings and improve the overall quality ratings?
  • 5Can you walk me through how your team coordinates care and documentation to prevent errors when a resident is transitioning between different medical needs?
  • 6Since therapy is a highlight here, what kind of social or recreational activities are available to keep residents engaged outside of their scheduled rehab sessions?

Key Review Excerpts

The care for my Mom from the Nurses, CNA’s. Therapy to food… everything was amazing! She was back to her prior level of functioning because of Timber Springs

Rehab patient family member · 2026★★★★★

If you are here every day as I am you would see that this facility is completely under staffed and has no ability to care well for most of the residents especially those here for skilled nursing. Response times for call lights average 20-60 minutes.

Long-term resident's family · 2025★★☆☆☆

This place is terrible, they released my dad with a stage 4 bed wound and gave no supplies or help for us to take care of him, he just went to the hospital yesterday he was so messed up.
