What this means for your family

This facility is highly recommended for short-term physical therapy, where the staff is consistently praised for their expertise and dedication. However, families of residents requiring high-level medical care or assistance with daily living should exercise extreme caution, as multiple reports of neglect, slow response times, and hygiene issues have been documented. We strongly recommend visiting during off-hours and weekends to observe the actual level of care provided.

Springfield Rehabilitation and Healthcare Center receives high praise for its physical and occupational therapy departments, with many residents reporting successful recovery outcomes and personalized care. However, the facility faces serious, recurring allegations of neglect, including slow response times for basic assistance, poor hygiene, and inadequate communication regarding medical care. Families should be aware of a stark divide between the positive experiences of short-term rehab patients and the critical concerns raised by families of long-term or high-acuity residents.

Questions for Your Tour

  • 1I noticed the physical therapy team is highly regarded here; how do you ensure that same level of dedication and attentiveness carries over to the daily nursing care and assistance with basic needs like toileting and feeding?
  • 2Given the recent focus on facility renovations, what is your current protocol for maintaining consistent hygiene and cleanliness standards within individual patient rooms and bathrooms?
  • 3With 15 recent state violations, what specific steps is the leadership team taking to improve oversight and ensure that all staff members provide respectful, professional care to every resident?
  • 4Could you walk me through your communication process during a medical emergency, and how you ensure families are kept informed and involved when urgent health changes occur?
  • 5I see you occasionally respond to feedback online; how do you use that input to address concerns regarding medication management and overall communication with families?
  • 6What kind of daily social activities or engagement programs do you have in place to ensure residents feel active and supported outside of their scheduled therapy sessions?

Key Review Excerpts

The therapy team here is amazing, they pushed me in every session to ensure I achieve my goals. Sam and Sean were my primary therapists and they were both amazing.

Rehab patient · 2024★★★★★

The in room bathroom, NOT CLEANED from Saturday to Wednesday. The soap dispenser did not work and the sink was clogged. Ask numerous times if they could fix it and it never happened.

Family member · 2025☆☆☆☆

My mom was here for 2 weeks in June 2025. The care was horrendous. She needed help eating and repeatedly asked somebody to cut her food and to sit her up so she can eat. They refused.

This facility shows a concerning pattern of repeated deficiencies, with over half triggered by family complaints about safety hazards, medication errors, and infection control problems. The most frequent issues involve accident prevention and safety supervision, infection control programs, and care planning, with safety violations appearing across multiple recent surveys. While the facility corrects most violations when cited, the high volume of complaint-driven inspections suggests ongoing quality concerns that families should carefully consider before placement.
