What this means for your family

This facility features a highly regarded rehabilitation team and active community engagement, which can be beneficial for short-term recovery. However, the recurring, serious reports of neglect and medication errors are a major red flag. If you choose this facility, we strongly recommend frequent, unannounced visits and active involvement in monitoring your loved one's daily hygiene and medication schedule.

South Ogden Post-Acute receives highly polarized feedback, with many reviewers praising the dedicated therapy team and helpful administrative staff, while others report severe concerns regarding neglect and poor hygiene. While some families feel the facility provides compassionate care and a welcoming environment, a recurring pattern of complaints highlights issues with medication administration, call light response times, and basic hygiene for residents. Families should be aware that experiences appear to vary significantly depending on the specific wing or staff shift.

Key Review Excerpts

The therapy team helped her reach her goals in the timeline she wished and got her back home with support services. As a family, we couldn’t be happier with the services provided.

Rehab patient's family · 2025★★★★★

I watched some amazing staff members come in and take care of her, but there were too many abhorrent staff members to counteract them. I am guilt ridden knowing that my grandmother was given terrible care in her final days.

Long-term resident's family · 2024☆☆☆☆

They never change his breif (one time there was poop runnin up his shirt and it also soaked his sheets) they said “we just checked him and he wasn’t wet” when you could clearly see they haden’t checked on him.
