What this means for your family

While the facility's therapy department receives consistent praise for its effectiveness, the recent surge in reports regarding neglect and safety lapses for high-needs residents is concerning. If you are considering this facility, we strongly recommend unannounced visits and asking specifically how they ensure 24/7 supervision and hygiene for residents who require assistance with daily living.

Key Review Excerpts

The PT with Katie and OT with Gabby worked wonders for him. I would like to mention the excellent care given to him by Gupta, Freida, Debbie and all of the other staff.

Rehab patient's spouse · 2026★★★★★

I was in this facility for only 60 minutes, and that was 60 minutes too long. The entrance is run-down and unwelcoming, leading to a dirty elevator with broken buttons. Upon reaching the second floor, the smell of urine and feces was so overpowering it took my breath away.

Family member · 2026☆☆☆☆

Unfortunately, they were always short-staffed, and often my mother was not gotten out of bed until the afternoon. Physical and Occupational therapy sessions were not consistent. Lastly, her clothes were either lost or misplaced.
