The facility failed to maintain the home in an uncluttered, clean, and orderly manner. The environment contained obstructions and hazards that violated housekeeping standards.

The facility failed to maintain a safe environment free of hazards due to a loose grab bar in the shower enclosure used by residents in rooms 121 and 122. An inspection revealed that one mounting plate on the upper L-shaped grab bar was dislodged from the wall, making the bar unstable when pulled.

The facility failed to provide the required written notice of discharge and appeal rights for a resident whose medical needs could no longer be met by the facility. Specifically, the facility did not ensure that the Adult Care Home Notice of Discharge and Hearing Request Form were properly delivered to the resident or their responsible person when the resident was not admitted back following a hospital stay.

The facility failed to provide the required Notice of Transfer/Discharge and ensure that discharge and appeal rights were based on a formal assessment or medical professional's recommendation for a resident whose medical needs could no longer be met by the facility. Specifically, for Resident #1, the facility did not document a change in level of care or a medical determination justifying the discharge to a hospital.

The facility failed to ensure that a resident received a required refund within 14 days after being discharged. Specifically, a refund for Resident #6 was paid 30 days after the resident's move-out date.

The facility failed to ensure that residents are properly refunded when they are notified of an intent to discharge. Specifically, the facility did not follow the requirement to refund the cost of care for the remainder of the month, minus any nights spent in the facility during the notice period, within 14 days of the resident's departure.

The facility failed to administer medications as ordered for one resident, specifically missing multiple doses of an anticonvulsant medication (divalproex) in September 2022. Staff documented missed doses due to waiting on pharmacy prescriptions but failed to notify the resident's primary care provider of the missed doses.

The facility failed to administer medications as ordered for one of five sampled residents. Specifically, the resident was missing prescribed anticonvulsant, antipsychotic, and sedative medications, as well as eye drops.
