The facility failed to ensure that one of three sampled residents had the required second step Tuberculosis (TB) skin test completed in compliance with guidelines. Specifically, Resident #2 was admitted in October 2020 and had a single TB skin test documented in December 2020, but no second test was recorded.

The facility failed to maintain clean and protected food storage and preparation areas. The refrigerator had surface rust and dirty door seals, while the top of the unit had a heavy buildup of grease and dust. Furthermore, cabinet shelves and doors were dirty with food particles and grease, and the oven interior was covered in blackened food debris.

The facility failed to keep walls, ceilings, and floors clean and in good repair. Observations revealed a worn, discolored carpet in the dining room, dusty walls and light fixtures in the common bathroom, and a rusted heating vent in a private bathroom. Additionally, a mirror was found with black mold between the glass and backing.
