Clean linens were stored improperly on a spa room counter and partition, and the clean linen storage area was being used to store Hoyer slings, linen carts, and a fan.
Kitchen supplies (bowls and disposable food items) were stored in the janitor's closet, which should only contain housekeeping items.
A bathroom in the memory care unit was being used for storage of dirty laundry, housekeeping supplies, and biohazard materials.
Resident soiled laundry was found in a storage closet that was not the designated soiled linen area.
Four employees were found to have completed tuberculosis screenings outside of the required 10-day window from hire.
Hazardous and toxic chemicals were found in an unlocked upper cabinet in the memory care unit and in the assisted living dining room cabinets.
