Recent inspections have identified concerns regarding the documentation of resident hygiene (showers) and the privacy of medical records on computer screens. Additionally, past inspections noted safety risks regarding loose bed rails and outdated resident service plans.

Failure to provide documentation that residents were receiving showers twice per week; five residents had no documentation of showers/refusals in July, and ten residents were missing one or two dates of documentation.
Sep 7, 2023Routine
Resident service plans were not updated to accurately reflect the use of assistive devices and receipt of hospice services.
Bed rails on 11 resident beds were loose with gaps large enough to be considered an entrapment risk; physician orders and service plans did not reflect the use of these devices.
Medication records were left open on a computer atop a medication cart in the memory care unit, exposing protected health information of all memory care residents.
