What this means for your family

While this facility is noted for accepting residents who may struggle to find placement elsewhere, the recurring reports of poor hygiene and neglect are serious red flags. We strongly recommend conducting an unannounced visit to inspect the cleanliness of the resident rooms and observing staff response times before making a decision.

Key Review Excerpts

My mother was here a few years back. I had to remove my mom due to neglect. Her room was always filthy every time I visited my mom smelled of dried urine.

Long-term resident's family · 2018☆☆☆☆

I think it's nice that this facility provides help for Men with felonies in their past, most Nursing Homes don't, making it very hard for families of these Men to find proper care for their loved ones.

Family member · 2024★★★★

1 complaint-triggered

This facility shows recurring problems across multiple areas that families should consider carefully. The most frequent issues involve fire safety and building systems (sprinkler maintenance, emergency lighting, fire doors appearing in multiple surveys), infection control appearing in three separate inspections, and medication management errors. While all 44 deficiencies across four surveys have correction dates, the pattern of repeated safety system failures is concerning. Additionally, one family filed a complaint that triggered an inspection revealing resident protection issues in 2025.
