  • 1With the Jasmine Place unit receiving such high praise for its leadership, could you walk us through how that team coordinates care and keeps families updated on their loved one's progress?
  • 2I noticed that response times to call lights have been a point of feedback; what systems or staffing adjustments do you have in place to ensure residents receive timely assistance when they need it?
  • 3Given the importance of safety during rehabilitation stays, what specific protocols do you follow to prevent falls and ensure skin integrity for residents who are less mobile?
  • 4We understand that communication is vital; how do you typically share updates with families, and what is the best way for us to reach out if we have questions about our loved one's care?
  • 5Since dining is such a central part of daily life, what steps are you taking to improve the meal experience and ensure residents are enjoying their food?
  • 6How does the facility handle medical emergencies or sudden changes in health, and at what point would we be notified by the nursing staff?

Key Review Excerpts

The staff at Jasmine Place are incredible. Their care and devotion to each resident is exceptional. Our family will always be grateful for the way they helped our mother find a sense of purpose, belonging, and comfort.

Memory care family member · 2025★★★★★

Beware if you have an elderly patient needing rehab! 93 year old completely blind and Confused patient left in room by herself with bed all the way up. Staff assumed it was her “normal.” She was found by staff underneath her bed after trying to go to the bathroom on her own and had fallen.

Rehab patient's family · 2025☆☆☆☆

The staff at the Jasmine Unit is phenomenal. The head nurse Amy is the most caring person I’ve seen work with Alzheimer’s patients. She models for the nurses that work there, and they are a reflection of her leadership.
