What this means for your family

While some residents report positive experiences with the therapy team and the 'small house' environment, the frequency of reports regarding medication errors and severe understaffing is alarming. If you consider this facility, you must conduct an unannounced visit on a weekend and specifically ask for the current nurse-to-resident ratio and their protocol for handling medication administration errors.

Key Review Excerpts

It took pulling and prauding at the employees to get his bandages changed. They couldn't provide frequent enough care for him. We got him out and had him transported to OSU. The OSU doctors said the wound care was done poorly and he had to go straight into surgery to repair them.

Family member of former resident · 2025☆☆☆☆

The small house concept is ideal for the needs for our loved one at this time.

Family member of resident · 2020☆☆☆☆

Short staffing at some times result in 45 minutes waits for help, the aides are great but can only do so much. Food carried to rooms is uncovered and are room temperature at most times.
