What this means for your family

While the facility has a highly visible and helpful concierge program, the recurring reports of slow response times and neglect are concerning. If you choose this facility, ensure you have a designated family member who can visit frequently to advocate for your loved one's basic needs, as the quality of care appears highly dependent on active family oversight.

Optalis Health and Rehabilitation of Troy receives highly polarized feedback, with many patients praising the concierge and therapy teams while others report severe neglect and communication failures. While the facility is frequently described as clean and welcoming by some, multiple families have raised critical alarms regarding slow response times to call lights, inadequate wound care, and poor medication management. Families should be aware that experiences appear to vary significantly depending on the specific staff on duty and the patient's individual needs.

Questions for Your Tour

  • 1Given that the facility has a high volume of residents, what specific protocols are in place to ensure that call lights are answered promptly, especially during evening and weekend shifts?
  • 2I noticed the facility has a very active therapy department; how do you coordinate the physical and occupational therapy sessions with the daily nursing care to ensure my loved one stays on track with their recovery?
  • 3What is your standard process for keeping family members updated on changes in medical status or medication adjustments, and who is our primary point of contact for these updates?
  • 4With the recent focus on facility cleanliness, could you walk me through the daily routine for room maintenance and hygiene checks to ensure residents remain comfortable and well-cared for?
  • 5Since your admissions and concierge teams are so highly regarded for their support, how does that team help bridge the gap between initial move-in and ongoing clinical care?
  • 6What steps does the nursing team take to ensure accuracy during medication administration, and how are errors or concerns addressed if they arise?

Key Review Excerpts

My mother has been here for a month and let me tell you Amy the concierge is such a wonderful coordinator helper. I would recommend her at all times they took care of her great facility great rehab therapy.

Rehab patient's family · 2024★★★☆☆

My dad was there from Thursday - Sunday before the staff neglect sent him back to the hospital. He received absolutely no rehab while there. He requested to be walked around, but by the time somebody arrived (on Saturday) his legs had atrophied.

Long-term resident's family · 2026☆☆☆☆

The nurses rolled their eyes if you asked for the smallest of things. Physical Therapy was pointless, if they even came at all. That would not give e time to expect when that would be here.
