My dad was there twice – he’s 99 and he fell and broke his femur and was discharged to ONAR —recovered there and went back to memory care and a week or so later fell and broke his hip and had a hip replacement and was once again transferred from the hospital to ONAR.

My husband is was there. He pushed the call button. Nobody responded. He got up on his own and fell. Took them 2 days to have x ray come. We had to do MRI and CT scan. Broken femur bone behind his left knee. NO INCIDENT REPORT.
