What this means for your family

Northbridge Health Care Center is frequently praised for its cleanliness and effective rehabilitation programs, which may be beneficial for short-term recovery. However, families should be aware of recurring reports regarding poor communication and serious lapses in patient care. We strongly recommend scheduling an in-person visit to observe staff-to-patient interactions and asking specific questions about their medication administration protocols and communication policies.

Questions for Your Tour

  • 1I noticed that your leadership team is very active in responding to feedback online; how do you use that family input to improve the daily quality of care here?
  • 2Given the recent focus on enhancing communication, what is the best way for our family to get timely updates on our loved one's status if we aren't able to reach the nursing station by phone?
  • 3With 145 residents in the facility, what specific protocols do you have in place to ensure medication management remains accurate and consistent for every individual?
  • 4We understand that dietary needs are complex; how do you track and verify that specific allergy requirements or menu preferences are consistently met during every meal service?
  • 5Could you walk us through the daily activity schedule and explain how staff encourages residents to participate in social life, especially for those who might be more reserved?
  • 6In the event of a medical change or emergency, what is your process for notifying family members and coordinating with our loved one's primary physician?

Key Review Excerpts

My dad has made so much progress here and I believe that Northbridge gave a piece of my dad back to me that I never thought I would see again!

Long-term resident's family · 2019★★★★★

Caught them giving us the wrong patients medication, (heart medication to a person with no heart issues). Takes hours to get help to go to the bathroom. They just let you soil yourself and get bed sores.

Family member · 2018☆☆☆☆

They never answer the phone, very blunt disrespectful social workers that will antagonize you, they’re not compassionate about loved ones.. I really feel the lack of support they had for my brother that passed away last week in this facility

Families have repeatedly filed complaints about this facility, with serious concerns about resident protection from abuse and neglect appearing multiple times from 2024 into 2025. The facility struggles most with resident safety and accident prevention, staff training and competency, and care planning quality. While all deficiencies show correction dates, the recurring pattern of abuse-related violations triggered by family complaints suggests persistent problems with fundamental resident protection that families should carefully investigate before choosing this facility.
