What this means for your family

While the facility's physical environment and therapy department are frequently praised, the recurring reports of critical medication errors and ignored call buttons are major red flags. If you choose this facility, we strongly advise daily visits to monitor your loved one's medication and hygiene, and to maintain direct contact with the administration to ensure care plans are being followed.

The Neighborhoods at Tiger Place presents a stark divide in experiences, with some families praising the facility's cleanliness and therapy team while others report severe neglect and safety concerns. A recurring pattern of complaints highlights chronic understaffing, slow response times to call buttons, and significant issues with medication management. Prospective families should be aware that while the facility is physically appealing, the quality of care appears highly inconsistent and potentially dangerous for residents requiring frequent attention.

Key Review Excerpts

My biggest concern is safety and patient care. While a resident my mom experienced missed medications, missed therapies, eating with plastic forks because they “run out” of metal forks daily, an undocumented fall

Rehab patient's family · 2026☆☆☆☆

The aides sit around a table in a great room basically ignoring the residents, there are no mentally stimulating activities, the residents have to sit in the same positions for hours on end in a large room with a TV blaring...it's a sad situation.

Family member · 2023★★☆☆☆

My dad’s care was acceptable until he was forced to move downstairs to the private-pay unit ($11K/month). Once there, the conditions and care were abysmal—so different it felt like an entirely separate facility.

This facility has concerning patterns with families filing multiple complaints, including serious issues like resident abuse, emergency response failures, and bed rail safety violations. The most recurring problems involve safety hazards, medication management, and infection control, with many issues persisting across multiple surveys from 2022 through 2025. While the facility corrects deficiencies when cited, the ongoing pattern of complaints and repeated violations in critical care areas suggests systemic quality concerns that families should carefully consider.

Try different approaches before using a bed rail. If a bed rail is needed, the facility must (1) assess a resident for safety risk; (2) review these risks and benefits with the resident/representative; (3) get informed consent; and (4) Correctly install and maintain the bed rail.
