Families have repeatedly filed reports about this facility, triggering over half of the 147 federal deficiencies found across 10 surveys. The facility has persistent problems with medication management, infection control, and ensuring a clean environment. Serious concerns include abuse reporting failures and multiple medication errors. While all issues show correction dates, the recurring nature of complaints and repeated violations in the same areas suggest ongoing care quality challenges that families should carefully consider.
