Key Review Excerpts

The nursing team was outstanding—always responsive, compassionate, and on top of everything. The rehab staff is highly skilled and truly dedicated to helping patients recover and regain independence.

Rehab patient family member · 2026★★★★★

My mother has acquired a stage 4 wound while being in the care of this horrid facility! Do not put your loved one here if u value their life!

Daughter of resident · 2026☆☆☆☆

The food is terrible, they do not cater to your requests if any. The staff lets patients sit in wet clothes do to no fault of the patient. Some of the care taker’s are rude, uneducated and no respect to your needs.

Meadow Park has had multiple families file complaints that led to federal citations, including serious issues with preventing abuse and neglect, and failure to timely report suspected incidents to authorities. The facility shows recurring problems across three main areas: fire and building safety violations, protection from abuse and neglect, and care planning. While all deficiencies show correction dates, the pattern of complaint-driven citations and repeat safety violations suggests ongoing operational challenges that families should investigate thoroughly before considering placement.
