Key Review Excerpts

The place is absolutely FILTHY. I visited my friend today to find BLEEDING SORES on his arms that weren't dressed. There were bugs on his bed.

Friend of resident · 2023☆☆☆☆

Most of the time when she buzzed for a nurse it sometimes took an hour sometimes longer..she was to have assistance to bathroom an to sit in wheelchair for her meals but put back to bed after,,.she was left in a wheelchair for 12 hours one day.

Family member · 2022☆☆☆☆

We moved our nonverbal mother here and there was little communication between the facility and hospice provider(Traditions). Both shifted blame to each other on various things.

This facility has significant concerns with 99 deficiencies across 22 surveys, including 44 complaint-triggered violations where families filed reports. While deficiencies appear to be corrected after identification, the pattern of repeated issues in the same care areas—particularly safety and accident prevention cited across multiple years—suggests ongoing challenges with maintaining consistent care standards.
