The facility used a transfer belt as a physical restraint to secure a resident to a wheelchair, which is contrary to the belt's intended use for transfers and walking. The belt was positioned across the resident's chest and arms rather than around the waist, and the physician's order for a waist belt was not being followed correctly. Additionally, the facility used the same type of transfer belt as a waist restraint for several residents.
