The facility failed to ensure that one of three sampled residents had a resident assessment completed annually. Specifically, Resident #3's care plan had not been updated since January 26, 2024.

The facility failed to ensure that the Primary Care Provider (PCP) signed and dated the resident's care plan within 15 days of the assessment. For Resident #2, the assessment was completed on March 24, 2024, but the PCP did not sign it until April 14, 2024.

The facility failed to ensure that participation by a licensed health professional, such as an occupational therapist, is provided within 30 days after admission and quarterly thereafter as required by the resident's health status.

The facility failed to ensure that one of three sampled residents had a resident assessment completed annually. Specifically, Resident #3's care plan and assessment had not been updated since January 26, 2024, despite an admission date of January 10, 2022.

The facility failed to ensure that the resident's physician authorizes personal care services and certifies the care plan by signing and dating it within 15 calendar days of completion.
