What this means for your family

While some families report successful rehabilitation outcomes, there is a recurring pattern of serious neglect and staffing issues. We strongly recommend visiting during off-hours or weekends to observe staffing levels and response times yourself, and specifically ask how the facility manages toileting assistance and hygiene for residents with mobility issues.

Key Review Excerpts

My mom was released to keystone after surgery for a ruptured appendix. She literally spent 22 hours there!! One LPN and one aid for 20 rooms and 2 patients in most rooms. I came to visit and she was sitting in a puddle of urine.

Rehab patient's family · 2022☆☆☆☆

He was well taken care of from the nurses, aides, therapy and the entire staff. He had mostly all the same caretakers day in and day out. How Keystone is able to hang on to the same dedicated staff (some that have been there many years) speaks volumes to their good reputation in itself.

Memory care family member · 2022★★★★★

My dad spent about 6 weeks here, the last 3-4 weeks being on the pathways hall (memory unit). On the best days, he received mediocre care. He was not fully incontinent before going to Keystone, but quickly became that way when he wasn't taken to the bathroom regularly.

Clean record — no deficiencies

This facility shows recurring safety and infrastructure issues, primarily with fire safety systems, electrical systems, and exit safety that have persisted across all three surveys from 2019 to 2024. While all deficiencies were corrected, the repeated violations of smoke barriers, fire alarm systems, and electrical wiring standards suggest ongoing maintenance challenges that families should discuss during visits to ensure resident safety systems are properly functioning.
