What this means for your family

While some families report positive rehab and hospice experiences, there is a recurring pattern of serious neglect and poor response times. We strongly advise families to conduct an unannounced visit during a weekend or evening shift to observe staffing levels and facility cleanliness firsthand before making a decision.

Key Review Excerpts

From the overwhelming smell of urine as you enter the place to the hallways they call rooms, to allowing residents to sit in their own soiled beds for HOURS.

Long-term resident's family · 2024☆☆☆☆

The care and compassion of the staff is outstanding. The only reason for not giving a 5 star rating is the building itself. It was a military barracks before and the concrete walls are a little industrial.

Long-term resident's family · 2025★★★★

He was comfortable, clean, & secure which allowed me to sleep nights, without fear, knowing He was not alone. His Nurse was so fond of Him, with always kind, & encouraging things to say.

2deficiencies
Well below state avg (7.2)

Holiday Heights Healthcare shows a concerning pattern of persistent fire safety deficiencies across all three surveys, with repeated violations in smoke barriers, emergency lighting, and sprinkler system maintenance. The facility also had recurring issues with resident assessments and food safety practices. While all deficiencies were reportedly corrected, the repeated fire safety violations across multiple years suggest ongoing challenges with maintaining critical safety systems that protect residents.
